CASE 2 흉통을 호소하는 60세 남자

C/C: Chest Pain
P/I
60세 남자로 내원 1일 전부터 시작된 retrosternal chest pain을 주소로 응급실 방문. 흉통은 쉬는 중에 발생하였으며 수시간 동안 지속되었고, moderate to severe 범위의 sharp pain이 오른쪽 어깨와 목으로 방사. 통증은 눕거나 심호흡을 하면 심해지고, 앞으로 기울이면 경감됨. 숨이하거나 심장이 두근대는 일은 없음. 최근에 호흡기 감염이 있었으나 2주 전 호전됨.
Past Hx
DM, HTN, hypercholesterolemia, adenocarcinoma of the prostate, costochondritis (2일전 진단)
Personal Hx
smoking: 1 pack of cigarettes a day for the last 20 years
medication: enalapril, hydrochlorothiazide, metformin, statin
Family Hx
형제 두명이 premature coronary artery disease

V/S BP 130/80mmHg HR 80bpm BT 37.0°C
Lab
normal CBC
creatine kinase 289 units/L (normal range, 0-200 units/L)
creatine kinase-MB (CK-MB) fraction 3.9 ng/ml (3.9 μg/L; normal range, 0-7.0 ng/ml)
undetectable troponin I (<0.02 ng/ml [0.02 µg/L])

상기 환자에서 가능한 진단은?
“Acute pericarditis”
This patient presented with retrosternal chest pain, which began while the patient was at rest and radiated to the right shoulder and neck. The pain worsened on deep inspiration and while lying down, but it improved when he would bend forward. In addition, a friction rub was detected on repeat examination. The initial ECG showed mild ST-segment elevation in the inferior and anterior leads that became much more pronounced over the following 12 hours, as well as PR-segment elevation in lead aVR.
These findings prompted the diagnosis of acute pericarditis.
참…내과가 아닌 다른 과 전문의가 답다는 것도 좀 이상하고…심히 쪽팔리는 일이겠지만….
랩이나 임상증세에서 AMI로 의심되는 소견이 없고, EKG에서는 잘 모르지만, ST depression도 없고…..특징적으로 앞으로 기대면 감소되는 통증과 2주전 호흡기 감염등….을 고려할때…..
심장 자체의 문제보다는 심근염쪽을 의심해야 될 것 같은데요.
근데 오른쪽 어깨와 목으로 방사하는 것으로 봐서는…..심장이 좌측이 아니라 우측?
그래서 결론은….우심증 환자에서 감기 바이러스로 인한 급성 심근염…..
틀리면 바로 지웁니다….쪽팔려서…
본의 아니게 선생님을 시험하게 되어버렸네요 ^^;; 앞으론 답이랑 같이 올려야겠어요. 그때 부연 설명 많이 해주세요!
